Prochaska and DiClemente's model describes several stages that a person goes through to effect change. For health care providers, this is extremely valuable as the stages then have corresponding strategies to use with patients to help them make the changes needed to improve their health. The model can be applied to smoking cessation, safe sex practices, diet, exercise, medication adherence, alcohol or drug abuse or any habit that is negatively affecting a persons health or mental health. Persons may not necessarily go through each stage progressively in a proscribed logical order, but may fall back to earlier stages or leap ahead to taking action depending on a number of factors. The attached table is a quick reference to the stages and provider strategies. (handout to attach) |
